You have reached the RETURNING STUDENT online enrollment form. Please follow the steps below prior to, & immediately following the completion of the online form.
- Access and read the Fatal Allergy Statement, Medication Guidelines, Transportation Statement, 19/20 Handbook, Bus Discipline Policy, Lunch Policy, and Internet Authorization Statement. These statements/policies are found on the Eufaula Elementary website es.eufaula.k12.ok.us under the Parent Resources tab at the top of the page. Please read these statements/policies BEFORE starting the online enrollment process! You will be asked to electronically sign the online enrollment form acknowledging that you have read the documents.
- Complete the returning student online enrollment form only if your child is a RETURNING student. Returning students are those who completed the previous school year at Eufaula Elementary. If your child attended the elementary but moved and finished the year at any other school, you will need to re enroll your child in person at the Elementary office. The online enrollment form will be emailed to the appropriate elementary office staff member for processing.
- To complete the returning enrollment process you must complete the current HEALTH HISTORY FORM and TRANSPORTATION ENROLLMENT FORM that will be sent home with your child during the first full week of school. When you recieve them, please complete them and return them to your child’s teacher or the elementary office on the next school day.
- Once you have competed steps 1 and 2, your returning child will be placed in their class. Class rosters will be posted at the Elementary Gym on MEET THE TEACHER night.
